Eligibility Criteria
General
a. Presence of a disabling condition that creates a barrier to employment of effective independent living.
b. Absence of an acute medical condition, emotional disturbance, or behavioral problem that would preclude participation in rehabilitation services; ability to benefit from services provided (not dangerous to self)
c. Ability to learn, as needed.
d. Availability of an attendant to provide care (toileting, feeding etc..) if the client is unable to provide his/her own self care.
e. Ability to conduct himself in a manner that allows others to benefit from services(not dangerous or highly disruptive to others.
f. Participates voluntarily.
Geographic
a. resident of the State of Ohio(or neighboring states and referred by an agency of the State of Ohio)
b. Has reliable transportation or living accommodations to receive center-based services or is within the travel distance of assigned staff for field services.
Ability to Pay
a. There is a source of payment for services (rare exceptions)
Collaborations (ongoing eligibility)
a. referral source allows client program to continue (if client was referred through another program, that professional may be in “control” of length of program, source of funds for payment, etc…)
